CSTE have agree to provide a temporary worker to fill vacancy Ref  code VA…………. 

 

By signing this form you are agreeing to treat the temporary worker with the same level of respect and consideration all other workers are accorded.

 

Timesheets: The temporary worker will be provided with a time sheet which will be completed weekly and will require a signature / email confirmation from a member of your staff. This is normally undertaken by the person named as the supervisor.

 

When you sign the temporary workers Time Sheet you are expected to check the hours worked to ensure they are correct. CSTE will charge you for all hours and part hours on each signed sheet. 

 

Expenses: Any agreed expenses incurred as part of the placement will be invoiced directly to the employer.

 

Insurance: Whilst engaged by your organisation, the temporary worker will be covered by your insurance policy.

 

Risk Assessment: Please ensure that you have carried out a risk assessment for this role. The work must be made fully aware of any risks associated with their duties during the induction process and prior to commencing work.

Please forward a copy of the relevant risk assessment to CSTE for attachment to insurance form for our records.

 

 

Fees: CSTE will charge £ 8.50 Per hour to provide a temporary member of staff to your Company / Organisation.

 

There is no affiliation fee for Green Stewards, unless a CRB check is required in which case the fee is £50 per person.)

 

Invoicing: You will be invoiced Monthly / Weekly for the time. Invoices will be sent by email to the address given in the placement agreement form, unless otherwise stated.

CSTE will endeavour to provide a replacement worker if the worker above is unwell or unable to work for any reason.

 

I agree to pay CSTE for all hours /part hours worked by the temporary worker.

 

If at any time during the placement or for a six month period following the placement we decide to offer the temporary worker a post paid directly by our payroll department we agree to pay CSTE a finders fee of £300.00.

 

I understand that the agreement may be terminated by either party with one week written notice.

KEY TERMS AND CONDITIONS – CLEAN SLATE TEMP WORKERS

You will be employed by Clean Slate Training & Employment (‘the Company’).  Outlined below are your main terms and conditions of employment as required under section one of the Employment Rights Act 1996. You will receive a contract to sign, which will include specific terms, such as rates of pay, etc.

 

If you change your name or address, you must inform a member of the Clean Slate team (and update your online profile) at the earliest opportunity. Not doing so may cause delays in offers of paid work placements and payment for work undertaken.

 

HOURS OF WORK, SALARY AND HOLIDAY PAY

You will be on a Zero Hours Contract, which means your hours may vary from week to week and there is no guarantee of work in any week. Your rate of pay is stipulated in your contract.  Pre-payment of your holiday pay will be made each time you undertaken paid work, as covered in your contract.  You will be paid weekly in arrears as per your formal contract.  Payment will be made directly into your bank or credit union account, which we can help you set up if you do not have one already.

You will need to sign a Code of Practice, if you have not done so already. This sets out what you can expect of Clean Slate as your employer and what we expect of you. It also asks you to agree to outline terms about:

Clean Slate has a full set of policies and procedures, which are set out in a staff handbook. You will have access to an online version (which is kept up to date) and can have a written copy at any time, upon request. (Your first

ABSENCE FROM WORK

If you are booked to work and cannot make it for some reason, you or someone on your behalf must inform a Clean Slate manager as early as possible – no later than one hour before you were due to start. Absence from work will mean you do not get paid and unexplained absence may mean you are less likely to be booked for work in future.  Please refer to our sickness policy for details about any entitlement to sick pay.

 

NOTICE PERIOD

Either party is required to give 1 weeks’ notice to terminate your contract of employment. The Company reserves the right to make payments in lieu of notice.

 

DEDUCTIONS FROM PAY

The Company will be entitled at any time during your employment and in any event on termination to deduct from your pay under any sums due from you to the Company such as outstanding loans, overpayments, advances, the cost of repairing any damage or loss to the Company’s property caused by you or excess holiday taken.  A thorough investigation will always be carried out.

 

RETURN OF COMPANY PROPERTY

On the termination of your employment you agree to give back all property belonging to Clean Slate and relating to the business of the Company.
